Our goal is to ensure your patient’s treatment is successful. At Vital Care of Crown Point we take a proactive approach to assist physician offices in lessening the paperwork and providing assistance in the care of your patient.
Provider Service
Our clinical staff will contact your patient to carefully explain every step involved in their therapy. This also gives the patient the opportunity to ask questions to a member of our clinical staff. We conveniently deliver your patients medicine to their home or your office free of charge.
Vital Care of Crown Point will be there for you and your patients, from the first day until the last day of your patient’s therapy. Our clinical staff will routinely call your patients to help achieve the best possible outcomes. We work closely with you to monitor compliance and adherence of each patient. Vital Care of Crown Point will always contact your patient before delivering their medications, and we will also remind you if a refill authorization is needed.
You can simply and securely fax us the prescription and demographic information for your patient or choose one of our easy to use enrollment forms. We will handle the insurance verification process for your patient’s medical and drug benefit. We assist with prior authorizations and any additional paperwork needed for the insurance company (when allowed by law or the insurer).
By offering 24/7 call support, Vital Care of Crown Point guarantees that providers and patients will receive assistance whenever necessary. Our clinical staff of pharmacists and nurses are always available to provide the best care for your patients.

Glenn Jabaay
Glenn brings extensive experience across retail, hospital, nuclear, long-term care, home infusion, and specialty pharmacy, and a wealth of knowledge to his role as Pharmacist-in-Charge at Vital Care of Crown Point. He is responsible for the overall operation and management of the pharmacy and ensuring that every patient receives safe, effective infusion medication therapy.
Glenn earned his BS in Pharmacy from Ferris State University and his PharmD from the University of Illinois at Chicago. His diverse background also includes time spent as a Competency Assessment Programs Manager at NABP, further sharpening his expertise in pharmacy practice and patient safety.
In his spare time, Glenn enjoys spending time with his wife and their four dogs, watching baseball and hockey, fishing, and exploring new travel destinations.

Scott Yilk, M.D
Co-owner of Vital Care of Crown Point. Dr. Scott has over twenty years’ experience in some of the busier Emergency Departments in the Chicago area and is currently an Emergency Room doctor in Naperville. He is also co-owner of IVmenow during its first month of launching in 2012 and has been integral to implementing and maintaining the systems, processes, and protocols for IVme’s growing menu of services. This business gave him a great deal of knowledge in infusion therapy. Dr. Scott graduated from Hastings College and the University of Nebraska Medical Center College of Medicine. He attended UIC residency for Emergency Medicine. He lives in Naperville with his wife and two daughters. In his free time, he enjoys travelling, golf, fishing and hunting.

Neil Morandi, M.D
Co-owner of Vital Care of Crown Point. Dr. Morandi is the Chief Executive Officer for IVme. Neil was co-founder of MedExpress Urgent Care, the largest urgent care company in the U.S., privately operating 141 centers in multiple states when it was acquired by Optum/United Healthcare in 2015. Neil is board-certified in emergency medicine, holds a BS degree in Biology from Indiana University of Pennsylvania and Doctoral of Medicine from Temple University School of Medicine, and completed his residency in Emergency Medicine from West Virginia University. Neil’s extensive knowledge and experience from growing a medical services organization from one to 141 centers is invaluable for Vital Care of Crown Point success, setting a course for continued growth, and fostering the right culture for employees and customer success. In his spare time, Neil enjoys fly fishing, golf, and cooking.

Joia D. Warner PharmD, MBA
Joia brings over a decade of extensive experience in pharmacy, her career showcases a proven ability to optimize patient care in diverse settings, from retail management, long-term care, to inpatient, specialty pharmacy and specialized oncology/infusion environments. My expertise extends to strategic planning, financial oversight, and staff leadership, consistently aligning clinical operations with business objectives while upholding the highest standards of patient safety and regulatory compliance.
Joia earned her Doctor of Pharmacy and Masters in Business Administration from Purdue University. Her diverse background includes time spent in retail and long-term care management, oncology specialty pharmacy, oncology outpatient infusion, as well as prior authorization and reimbursement specialty.
In her spare time she enjoys spending time with her daughter and dog alex, attending sporting events, serving at her church and golfing.

Nyla Alexopoulos | Pharmacy Tech
Nyla is a certified pharmacy technician with over five years of diverse experience across retail, inpatient, and compounding pharmacy settings. She has also served as a Medication History Specialist, where she developed a strong eye for detail and a deep commitment to patient safety and accuracy. Her well-rounded background allows her to adapt seamlessly in fast-paced healthcare environments while maintaining a patient-centered focus.
Nyla holds a Bachelor’s degree in Business Administration from University of Illinois Chicago and an Associate of Science degree from Moraine Valley Community College. Her academic foundation in business, combined with her clinical expertise, gives her a unique perspective on both operational efficiency and quality patient care.
Outside of work, Nyla enjoys expressing her creativity in the kitchen through cooking and baking, spending meaningful time with family, and traveling whenever she gets the opportunity to explore new places and experiences.
